We will bill your insurance, but we need a completed patient registration form and a copy of your insurance card.

If you recently changed insurance providers and have not received a card, our staff will attempt to verify the information with your carrier. Co-payments are expected to be paid at the time of service. We accept cash, checks, Visa and Mastercard.
